Gen-M 30mg Ampule 2 ml belongs to a group of medicines called antimalarials. Gen-M 30mg Ampule 2 ml is used for the initial treatment of severe malaria. Gen-M 30mg Ampule 2 ml comes in injection form. It can be used in adults and children. 

Gen-M 30mg Ampule 2 ml is an artemisinin derivative. Gen-M 30mg Ampule 2 ml hinders the normal function of Plasmodium parasites. Gen-M 30mg Ampule 2 ml has a short duration of action due to its short life. Gen-M 30mg Ampule 2 ml may cause side effects such as renal failure, hemoglobinuria, and jaundice. 

Indications

Gen-M 30mg Ampule 2 ml can be used for:

  • Initial treatment of severe malaria
  • Cerebral malaria

  • Second-line treatment in chloroquine-resistant falciparum malaria

How Does Gen-M 30mg Ampule 2 ml Work

Gen-M 30mg Ampule 2 ml is an antimalarial drug. Gen-M 30mg Ampule 2 ml is an artemisinin derivative that is rapidly metabolized to active dihydroartemisinin. Dihydroartemisinin generates free radicals to inhibit the normal function of plasmodium parasites.  

How To Use Gen-M 30mg Ampule 2 ml

Gen-M 30mg Ampule 2 ml comes in injection form. Gen-M 30mg Ampule 2 ml can be given via intramuscular or intravenous route. For the IM route dissolve the product with 1 mL of 5% sodium bicarbonate and inject. For slow IV infusion, dissolve Gen-M 30mg Ampule 2 ml with 1 mL of 5% sodium bicarbonate and add 5 mL of 0.9% sodium chloride and inject. Injection should be injected immediately, if the solution appears cloudy or sedimentation occurs the injection should be rejected. 

Dosage

The recommended dosage:

Adults

Severe malaria

2.4mg/Kg at 0, 12, and 24 hours then 2.4mg/Kg daily for up to 7 days. 

Reconstitution Method Gen-M 30mg for Injection

Step 1: Add 0.5ml Sodium Bicarbonate Inj. 5% in vial & mix well until clear solution. Step 2: For I.V use: Add 2.5ml Sodium Chloride Inj. 0.9% in the vial (Step 1) & mix well & use by slow I.V route over 2-3 minutes (Do not put the solution in an I.V drip).

For I.M use 

Add 1 ml Sodium Chloride Inj. 0.9% in vial (Step 1) & mix again & use by I.M route. 

Gen-M 60mg for Injection

Step 1: Add 1ml Sodium Bicarbonate Inj. 5% in vial & mix well until clear solution

Step 2: For I.V use: Add 5ml Sodium Chloride Injection 0.9% in the vial (Step 1) & mix well & use by slow I.V route over 2-3 minutes (Do not put the solution in an I.V drip). 

For I.M use: Add 2ml Sodium Chloride Inj. 0.9% in vial (Step 1) & mix again & use by I.M route. 

Gen-M 120mg for Injection:

Step 1: Add 2ml Sodium Bicarbonate Inj. 5% in vial & mix well until clear solution. 

Step 2: For I.V use: Add 10ml Sodium Chloride Inj. 0.9% in the vial (Step 1), mix well & use by slow I.V route over 2-3 minutes (Do not put the solution in an I.V drip). 

For I.M use: 

Add 4ml Sodium Chloride Inj. 0.9% in the vial (Step 1), mix again & use by I.M route.

Children

  • Age 7 years or more same as adult dose

  • Below 7 years: 1.5 mg/kg

  • In both the age groups children should be given the second dose after 4-6 hours then one dose after every 24 hours once a day.

Pregnancy and Lactation

Parenteral Gen-M 30mg Ampule 2 ml should be used for the treatment of severe falciparum malaria only where there is evidence that the antimalarial efficacy of quinine is declining. Gen-M 30mg Ampule 2 ml should not be used during the first trimester of pregnancy. The effect of Gen-M 30mg Ampule 2 ml on lactation is undetermined and precaution is advised. 

Contraindications

Inform your doctor if you have a history of any of the following before using Gen-M 30mg Ampule 2 ml:

  • Hypersensitivity to Gen-M 30mg Ampule 2 ml or its active ingredients
  • Electrolyte disturbances

  • Concomitant use with other drugs that cause QT-interval prolongation

Precautions

Inform your doctor about any of the following conditions before using Gen-M 30mg Ampule 2 ml

  • If you are pregnant or breastfeeding

  • Post-treatment hemolysis

  • History of arrhythmias

  • Clinical bradycardia

  • Congestive heart failure

  • Reduced ventricular ejection fraction

Interactions With Other Medications

Inform your doctor about the use of the following before using Gen-M 30mg Ampule 2 ml

  • Antioxidant drugs e.g., vitamin E, C

  • UGT inhibitors e.g., axitinib, vandetanib, imatinib, diclofenac.

Adverse Effects

Like most medicines,Gen-M 30mg Ampule 2 ml has certain side effects which include

  • Transient and reversible reticulocytopenia

  • Severe hemolytic anemia

  • Renal failure

  • Jaundice 

  • Drug fever

  • Rashes

  • Bradycardia

  • Transient first-degree heart block

  • Transient and reversible elevation of serum transaminases

  • Nausea

  • Abdominal pain

  • Diarrhea

  • Headache 

How To Cope with Adverse Effects

Nausea- To avoid nausea, take simple and small meals. Avoid rich and spicy food items. Rather than one large meal takes several small ones. Avoid high sugar content foods. 

Headache- Drink plenty of water and fluids and ask your doctor for a painkiller. If the headaches persist, then contact your doctor.

Constipation- Eat a well-balanced diet and drink plenty of water to avoid constipation. Add fiber to the diet to avoid constipation. 

Anemia- Pale skin, fatigue, weakness, and disturbed appetite are signs of anemia. To cope with anemia, eat iron-rich food such as beef, salmon, cereals, liver, dry fruits, and leafy vegetables. Take a good night’s sleep, take rest when needed to avoid fainting, and light exercise daily improves blood flow. Your doctor might order a blood screening test. Severe conditions may require blood transfusions.

Drug-induced fever- At time Gen-M 30mg Ampule 2 ml causes fever. Take an over-the-counter antipyretic. If fever persists, contact your doctor.
