{{Product}} is an antiemetic drug that is majorly used to treat nausea and vomiting associated with Gastroesophageal Reflux Disease (GERD), chemotherapy, and Gastroparesis (slow stomach emptying in diabetics). {{Product}} is also prescribed in the management of migraine, and administered along with pain killers to increase their efficacy.

{{Product}} works by increasing gastric motility in the upper intestinal region and thereby raises the gastric emptying rate. The prokinetic (control of gastroesophageal reflux) effect takes place by blocking dopamine (a neurotransmitter that induces nausea and vomiting). 

There is a warning associated with Metomide 5mg/5ml Syrup 50 ml that is “Tardive Dyskinesia”. {{Product}} should not be taken for more than 12 weeks to minimize the risk of developing Tardive Dyskinesia – a neurological disorder characterized by involuntary jerky movements of muscles, specifically in the head and neck region. Due to the risk of this serious side effect, Metomide 5mg/5ml Syrup 50 ml is not recommended to be given to children who are younger than 1 year of age.

How Does Metomide 5mg/5ml Syrup 50 ml Work

{{Product}} inhibits Dopamine (D2) and Serotonin (5HT-3) receptors in the brain (at the chemoreceptor trigger zone – responsible for emesis). Along with this, Metomide 5mg/5ml Syrup 50 ml acts as an agonist to 5HT-4 receptors that in turn increases the release of acetylcholine. This leads to a rise in gastric transit through the gut and the gastric emptying action takes place by lowering the pressure of the lower esophageal sphincter. This action doesn’t involve any gastric secretions. This way, {{product}] acts as a peristaltic stimulant and produces anti-emetic effects. 

Dopamine is responsible for relaxing the gastric smooth muscles. Metomide 5mg/5ml Syrup 50 ml opposes this effect by inhibiting the dopaminergic effect and increasing the cholinergic effect. The relaxing action takes place at the same time at the upper region of the small intestine so that the antrum and upper region of the small intestine coordinate in lowering the reflux. 

How To Use Metomide 5mg/5ml Syrup 50 ml

• Take the suggested dose of Metomide 5mg/5ml Syrup 50 ml from a syringe (for accuracy) or teaspoon as per the physician’s recommendation. 

• {{Product}} is normally administered half an hour before a meal and at bedtime as per the doctor’s instructions. 

• Do not take Metomide 5mg/5ml Syrup 50 ml in larger doses than recommended as there is a risk of developing a movement disorder as an adverse effect associated with long-term use or overdose of Metomide 5mg/5ml Syrup 50 ml. 

Dosage

Form and strength:

• Syrup: 5mg/5ml

• Tablet: 10mg 

• IV and IM injection: 10mg/2ml 

Adult Dose

10mg (2 teaspoons full) three times a day is the recommended dose generally given to adults and the duration of treatment is 5 days. The dose should be reduced for adults who weigh less than 60 kgs and for those who have significant renal or hepatic diseases. 

Diabetic Gastroparesis

10mg after every six hours (2 teaspoons full four times a day) 30 minutes before meals and at bedtime for 2 weeks to 8 weeks, depending on the patient’s condition. 

Gastroesophageal Reflux Disease

10 to 15 mg (2 to 3 teaspoons full respectively) after every six hours, 30 minutes before meals, and at bedtime, depending on the response to the drug. The dose should not be more than 80 mg in one day and treatment should not be exceeded to more than 12 weeks. 

Pediatric Dose

• {{Product}} is not recommended for use in children below 1 year of age.

Prevention Of Delayed Chemotherapy-Induced Nausea and Vomiting

0.1 to 0.15 mg/kg is the recommended dose that is repeated up to three times a day.

Pregnancy And Lactation 

{{Product}} crosses the placenta so it can cause extrapyramidal symptoms and methemoglobinemia in the infant. Breastfed neonates have gone through stomach upsets whose mothers were on Metomide 5mg/5ml Syrup 50 ml. The infant should be closely observed for any signs of dystonia (twitching of muscles) and other significant clinical signs which may reflect adverse reactions. 

Contraindications

{{Product}} is contraindicated or in the following cases:                 

• Known hypersensitivity to the excipients or the active agent.  

• Within the first 4 days of certain procedures like gut anastomosis and Pyloroplasty (a surgery in which the lower region of the stomach is operated to make it wide). 

• Perforation or active bleeding in the G.I tract. 

• In children younger than 1 year of age due to the risk of extrapyramidal disease (involuntary movement of muscles). 

• Pheochromocytoma (tumor of the adrenal gland) can lead to serious hypertension. 

• History of epilepsy or Parkinson’s disease.

• History of inherited blood diseases like Methamoglobinaemia, or NADH cytochrome B5 deficiency. 

• If you are taking red yeast rice as Metomide 5mg/5ml Syrup 50 ml increases its effects and there is a risk of myopathy. 

• If you are on antipsychotics as there is a risk of extrapyramidal effects (movement disorder).  

Precautions

Following are the points your doctor should know before starting the treatment with Metomide 5mg/5ml Syrup 50 ml:

• If you are pregnant or trying to get pregnant, or if you are a breastfeeding mother as {[product}} crosses the placental barrier. The infant should be monitored for the potential adverse effects if the mother has to take Metomide 5mg/5ml Syrup 50 ml.
