Heparin 25000IU Injection 5 ml belongs to a class of drugs called “anticoagulants”, commonly referred to as blood thinners used for the prevention and management of blood clots. In patients with heart and lung conditions, it helps decrease the chances of having a heart attack or stroke by easing out the blood flow and circulation through blood vessels. Conditions benefiting from Heparin 25000IU Injection 5 ml include venous thrombocytosis, pulmonary embolism, heart surgery, atrial fibrillation, and patients with chronic health conditions undergoing surgery or dialysis. It can be used alone or combined with other drugs as part of the treatment regimen. 

Heparin 25000IU Injection 5 ml is chemically composed of Heparin, which works by acting in the blood, preventing the formation of clots, or discouraging the size increase of previously formed clots in the blood. It does so by enhancing the effects of anti-thrombin 3, a protein that is naturally designed to block factors that aid in clotting (especially Thrombin, Factor Xa ). The clotting pathway generally takes a slow natural course, but it can be boosted via the injection of anti-coagulants. How to use Heparin 25000IU Injection 5 ml

It is given as a subcutaneous injection or injected into the veins by a healthcare professional, as prescribed by your doctor. Dosage is completely dependent on the weight, age, severity of infection, patient’s response to the medicine, and their comorbidities. Your doctor may vary your dosage accordingly. A new, sterile syringe must always be used and properly discarded following universal protocol for sharps disposal. Always start by cleaning your hands and the area to be injected. Carefully first check the glass vial to rule out any particulate matter in the solution. 

When Heparin 25000IU Injection 5 ml is to be administered frequently, the “at-home” subcutaneous injection technique can be taught to patients and healthcare providers. IV Heparin is administered in a healthcare facility as a quick injection or a slow infusion via a maintained IV line. Dosage

Adult Dose

DVT or Pulmonary Embolism

Prevention 

5,000 units subcutaneously at least 2 hours before your surgery followed by 5,000 every 8-12 hours subcutaneously for 1 week or till complete mobilization

Or 7,500 units subcutaneously every 12 hours

Treatment

Loading dose: 5,000 units intravenously (in severe pulmonary embolism 10,000 units may be required)

Maintenance Dose: 1,000 – 2,000 units/hour by intravenous infusion, or 10,000-20,000 units to be given at 12 hourly intervals via subcutaneous injection, 

Alternatively, 5,000-10,000 units at 4-hourly via intravenous injection

Acute Coronary Syndrome

Percutaneous Coronary Intervention: 

  • Without GPIIb/IIIa inhibitors like abciximab – an initial IV bolus of 70 – 100 units/kg
  • With GPIIb/IIIa inhibitors – an Initial IV bolus of 50 – 70 units/kg

ST-Elevation myocardial infarction (STEMI): 

  • Patient on fibrinolytic drugs: IV bolus of 60 units/kg (maximum dose: 4000 units), followed by 12 units/kg/hr. (maximum dose :1000 units/hr.) as continuous IV infusion

Unstable Angina: 

  • Initial IV bolus of 60-70 units/kg (max: 5000 units), followed by IV infusion starting with  12-15 units/kg/hr. (max: 1000 units/hr.)

Anticoagulation Therapy:

IV injection

  • Starting: 8000-10,000 units IV, followed by 50-70 units/kg (5000-10,000 units) every 4-6hr

Continuous infusion (IV)

  • 5000 units IV injection, followed by continuous IV infusion of 20,000-40,000 units/24 hr.

Catheter Rinsing:

Use 100 units/mL; add adequate volume to fill and flush the lumen of the catheter

Child Dose

It should not be given to neonates and infants, especially premature due to the high risk of adverse effects. 

DVT or Pulmonary Embolism

Loading dose: 50 units/kg intravenously

Maintenance Dose: 15-25 units/kg/hour by IV infusion, or 250 units/kg 12 hourly subcutaneously injection or 100 units/kg every 4 hours by intravenous injection 

Acute coronary syndrome

Loading dose: 50 units/kg intravenously

Maintenance Dose: 15-25 units/kg/hour by intravenous infusion, or 100 units/kg 4-hourly by intravenous injection

Elderly dose

The safety concerns of Heparin 25000IU Injection 5 ml in the elderly population may require careful monitoring of liver function and other factors. Advised doses are lower than the adult doses with careful monitoring of a PTT, PT, and clotting time. Your doctor should set the most appropriate dose for you based on your medical history. Doses are adjusted according to a liver impairment or other anti-clotting medications being taken concurrently, and the duration of the treatment. 

*Note: Regular monitoring of clotting and bleeding times are advised after starting the treatment with heparin. Dose adjustment is done accordingly

Pregnancy use and lactation

Heparin 25000IU Injection 5 ml is FDA pregnancy risk category C which has shown adverse effects on the unborn baby in studies and trials conducted on animals, therefore if you are pregnant, think you may be pregnant, or are planning to conceive a baby, please consult your doctor. Contraindications

How to cope with some of the adverse effects

  • Bleeding more easily than normal – Be cautious of taking part in activities that could result in an injury or a deep cut. Always wear a helmet while cycling, gloves while using sharp objects like scissors and knives, use an electric razor instead of wet shaving, a soft toothbrush while brushing, and waxed dental floss. Contact your doctor if more than normal bleeding occurs. 
  • Soreness at the site of injection- a cold compress can be used for 5 minutes to ease out the soreness.
  • Diarrhea – Keep yourself hydrated by having lots of fluids and if the problem persists, consult your doctor before taking any medication for vomiting and diarrhea
  • Indigestion/heartburn – Avoid food items that cause wind. Try having smaller and more frequent meals.
  • Stomachache – Try to rest as much as possible and have smaller and more frequent meals. Using a heat pack or hot water bottle over the area in pain might help in giving some relief. If the pain persists and is severe, speak to your doctor
  • Headaches – In which case make sure you rest and drink plenty of fluids. Avoid the use of alcohol. Take a prescribed painkiller. Consult your doctor if the headaches last longer than a week or are severe

  • Muscular Aches and Pains- A hot or cold compress can be placed at the area of discomfort to bring relief; an over-the-counter pain reliever such as paracetamol or ibuprofen can be ingested to bring ease as well. Avoid high-intensity exercise while suffering from muscular pain.
  • Constipation- Can be combated by adding more fibers to the patient’s diet, asking the patient to drink plenty of fluids, and increasing their physical activity to trigger contractions in the intestines.
