Chronic Kidney Disease on Dialysis
Hemodialysis

Dose adjustment: Increase by 25 IU per kg, 3 times a week every 4 weeks
Maintenance Dose: 75 to 300 IU per kg per week in 3 divided doses
Peritoneal Dialysis
50 to 100 IU per kg of body weight, given as a subcutaneous (SC) injection or an IV injection 3 times a week
Dose adjustment: Increase by 25 IU per kg, 2 times a week every 4 weeks
Maintenance Dose: 25 to 50 IU per kg per week in 2 divided doses
Anemia Associated with Chemotherapy
Starting Dose: 150 units/kg either IV or SC thrice a week
OR,
40,000 units SC once weekly until termination of chemotherapy
Target Hb: 10 to 12 gm/dL
Dose Adjustment
Reduction or increase is indicated by evaluating Hb levels every 4 weeks
Child Dose

Hemodialysis
