Top Plus Gel 30 gm belongs to a class of topical medicines which is a combination of synthetic steroid and aminoglycoside antibiotics. Top Plus Gel 30 gm is prescribed for the treatment of several inflammatory skin conditions, suited for superficial skin conditions with an ongoing bacterial infection as well such as those caused by eczema, dermatitis, psoriasis, infected wounds, etc. it is mainly used when other topical medicines fail to treat infectious inflammatory skin conditions. Vulnerable bacterial species include mild to moderate infections due to both gram-positive cocci and other bacteria including staphylococcus aureus, Proteus Vulgaris, and some other sensitive strains.

Top Plus Gel 30 gm contains Fluocinolone Acetonide and Neomycin which acts by reducing inflammation exhibited by swelling and redness, as well as inhibiting protein synthesis in susceptible bacteria and disrupting the cell membrane. How Does Top Plus Gel 30 gm Work?

Top Plus Gel 30 gm is more suited for inflammatory skin conditions with a concurrent bacterial infection. It works by acting on the skin via two basic mechanisms. Firstly, Fluocinolone being a corticosteroid controls the production of proteins that lead to inflammation by binding to Glucocorticoid receptors while also exerting vasoconstrictive action on blood vessels and controlling nitric oxide production, responsible for promoting inflammation. Also, it controls the infiltration and migration of inflammatory cells like leukocytes. It also inhibits Phospholipase A2 which speeds up the process of prostaglandin formation, a key mediator in inflammatory reactions. Top Plus Gel 30 gm thus decreasing the inflammation. Secondly, Neomycin via its bactericidal effects through cell membrane disruption and impaired protein synthesis inhibits the production of bacterial proteins thereby killing bacteria.

How to use Top Plus Gel 30 gm?

Top Plus Gel 30 gm is available in the form of ointment and cream meant for external use only to be applied with clean hands usually three to four times daily after washing the affected skin properly and gently drying afterward. Top Plus Gel 30 gm is safe for use in adults and children above 2 years of age. The doctor must always be consulted first when using Top Plus Gel 30 gm in children, careful application and monitoring are thus advised to avoid accidental ingestion. Fluocinolone is a steroid, thus a prolonged application is not advised.

  • Wash your hands after application, an additional amount can be reapplied if Top Plus Gel 30 gm gets wiped off from any part.

Dosage

Top Plus Gel 30 gm is not used for very long periods. Treatment is commonly prescribed for 7-10 days, if there’s no improvement in your symptoms after 7 days of application, consult your doctor who may consider changing the medicine for you. Adult Dosage

A thin film of Top Plus Gel 30 gm is to be applied two to four times a day, or as instructed by a physician.

A small amount on the fingertips should be gently rubbed in. Application on the face is not advised for more than 5 days.

Child Dosage

Top Plus Gel 30 gm must not be used on children without consulting a doctor. It is not considered safe for application in patients aged 2 years and younger. Top Plus Gel 30 gm is not recommended for use for more than 7 days.

Smaller amounts should be applied to a child’s body due to height and physique differences. Moreover, infants and children are likely to absorb more through the skin which can potentially result in side effects.

Commonly a pea-sized quantity on the fingertip is sufficient to cover the affected skin. Do not use Top Plus Gel 30 gm to treat diaper rash in infants.

Elderly Dosage 

For elderly patients the dosage of the drug may be lower than normal due to decreased kidney function, although Top Plus Gel 30 gm is absorbed through the skin in insignificant amounts but may pose a problem in elderly patients so, in such cases the doctor may prescribe an altered dose of the Top Plus Gel 30 gm.

Pregnancy use and lactation

  • Headaches – In which case make sure you rest and drink plenty of fluids. Avoid the use of alcohol. Take a prescribed painkiller. Consult your doctor if the headaches last longer than a week or are severe
  • Feeling or being sick (nausea or vomiting) or diarrhea – Avoid eating rich or spicy food. It might help to take medication after food. If you’re sick, try drinking small, frequent sips of water. 
  • Feeling dizzy (lightheaded) or weak – If the medication makes you feel dizzy or weak, stop what you’re doing and sit or lie down until you feel better, and do not rise too fast or immediately from sitting or lying down position to a standing posture.   • Photosensitivity – patients are asked to wear sun-protective clothes when going outside including gloves and a hat, to apply generous amounts of broad-spectrum sunblock along with wearing sunglasses. Patients are advised to do all necessary work after sunset to reduce or eliminate sun exposure while undergoing treatment with Top Plus Gel 30 gm.
