- Complicated UTI/Pyelonephritis:
- 1-17 years, 10-20 mg/kg twice daily for 10-21 days. Max 750 mg.
- Inhalation anthrax:
- 10-15 mg/kg twice daily for 60 days after confirmed or suspected exposure. Max: 500 mg/dose.
