Pulmonary arterial hypertension (PAH) is a diverse medical condition that may be due to several reasons including chronic lung or heart disease, connective tissue disorder, or pulmonary thromboembolism, the common outcome of which is stricture of blood vessels causing the buildup of pressure inside, manifested as high blood pressure and difficulty in breathing even on mild physical exertion. Bosmon 62.5mg Tablet 7 ‘S is a drug that belongs to a class of medicines known as endothelin 1 antagonist. This drug is prescribed to patients with high blood pressure resulting from PAH which not only relieves blood pressure but also improves other symptoms resulting due to PAH such as limited physical labor. 

Bosmon 62.5mg Tablet 7 ‘S works by blocking the action of endothelins, peptides with strong action on blood vessels whose levels are found to be high in patients with PAH. When endothelin binds to their most abundant receptors in the lungs, they bring about changes in the blood vessels causing them to narrow down which creates pressure inside the vessels manifested as high blood pressure. Moreover, it also causes constriction of airways (bronchioles) which makes it difficult for patients to breathe. By antagonizing the action of endothelin all this is reversed, lowering blood pressure and opening up airways for easy breathing.

Bosmon 62.5mg Tablet 7 ‘S is available in form of oral tablets to be swallowed with water two times a day, with or without a meal. The starting doses are kept low and increased after an observation period of four weeks to a higher dose. Bosmon 62.5mg Tablet 7 ‘S is not considered safe for pregnant and lactating mothers but safely prescribed to children 3 years and above. Contraception is advised when you are using Bosmon 62.5mg Tablet 7 ‘S to avoid getting pregnant. Abrupt cessation of the drug is not advised. Most common side effects observed with Bosmon 62.5mg Tablet 7 ‘S include dizziness, flushing, cough, and chest pain among others.

How Does Bosmon 62.5mg Tablet 7 ‘S Work?

Bosmon 62.5mg Tablet 7 ‘S is chemically composed of Bonesman, and endothelin 1 antagonist. It shows its therapeutic effects by blocking the action of endothelins, peptides with strong action on blood vessels commonly occurring in 3 different forms scattered through different tissues in the lungs, kidneys, heart muscles, etc. their levels are found to be high in patients with PAH. When endothelin binds to their most abundant receptors in the lungs, they bring about changes in the blood vessels causing them to narrow down which creates pressure inside the vessels manifested as high blood pressure. Moreover, it also causes constriction of airways (bronchioles) which makes it difficult for patients to breathe. How to use Bosmon 62.5mg Tablet 7 ‘S?

Bosmon 62.5mg Tablet 7 ‘S is available in the form of oral tablets to be swallowed with a glass of water through the mouth with or without a meal, twice daily. Taking Bosmon 62.5mg Tablet 7 ‘S with a meal is optional. The dosage at the beginning is set as the lowest available potency (62.5 mg) which is increased to the maximum safe dose as per need after 4 weeks. When treatment is intended to stop, the dose is cut down to an incremental dose of 62.5 mg over 1 week before stopping it completely. 

It is advised that the drug is taken at the same time each day, an alarm can be placed to remind patients to take Bosmon 62.5mg Tablet 7 ‘S at the proper time each day. If the patient misses a dose of Bosmon 62.5mg Tablet 7 ‘S, they should take it immediately but if the time for the next dose of the drug is near then it’s beneficial for patients to skip the missed dose and take the tablets as per schedule, doubling of doses has proven to cause severe side effects.

Dosage

Adult Dosage

One tablet of Bosmon 62.5mg Tablet 7 ‘S is prescribed to be taken orally twice daily (morning and evening)

The starting dose is on the lower side: 62.5 mg

After a therapy period of 4 weeks, 

Maintenance Dose: 125 mg to be taken orally twice daily (morning and evening)

Child Dosage

Children above 3 years of age and adolescents can be given Bosmon 62.5mg Tablet 7 ‘S in the following recommended dose. Always consult a doctor for the right amount you can take.

3 to 12 years (per kg of body weight):

4 kg to 8 kg: 16 mg orally twice a day

Greater than 8 kg to 16 kg: 32 mg orally twice a day 

Greater than 16 kg to 24 kg: 48 mg orally twice a day

Greater than 24 kg to 40 kg: 64 mg orally twice a day

*Following 4 weeks, the maintenance dose of each category (by weight) remains the same as starting dose

Above 12 years of age (per kg of body weight):

Less than 40 kg: 62.5 mg orally twice a day

Greater than 40 kg: 62.5 mg orally twice a day for 4 weeks, followed by a maintenance dose of 125 mg orally twice a day

Elderly Dosage

For patients aged 65 years and above, due to decreased kidney and liver function, Bosmon 62.5mg Tablet 7 ‘S may not be effectively removed from the body and so, in such cases the doctor may prescribe an altered dose of the Bosmon 62.5mg Tablet 7 ‘S which must be strictly followed.

Pregnancy use and lactation

Bosmon 62.5mg Tablet 7 ‘S falls under FDA pregnancy category X which means it should not be taken during pregnancy due to potentially harmful effects on unborn babies. Therefore, if you are pregnant, or are planning to conceive a baby, please do discuss Bosmon 62.5mg Tablet 7 ‘S with your doctor.

• ‌Bosmon 62.5mg Tablet 7 ‘S is likely to cause damage to your liver, based on your Liver function tests if there is an indication of an already compromised function, ‌Bosmon 62.5mg Tablet 7 ‘S must be avoided. Baseline levels are determined before the treatment (check aminotransferase) to determine the suitability of this drug. These levels should be checked regularly every month
• Signs of liver injury should be kept under check such as pale skin, vomiting, loss of appetite, a general feeling of sickness and fatigue
• Pregnant females or those trying to conceive must not use ‌Bosmon 62.5mg Tablet 7 ‘S. Strict contraception is advised while under treatment with ‌Bosmon 62.5mg Tablet 7 ‘S up to at least 1 month after the end of therapy. The contraception method must be reliable without fail, ask your doctor
• Frequent pregnancy tests at home (every month) must also be conducted to rule out the condition
• ‌Bosmon 62.5mg Tablet 7 ‘S can cause a drop in blood hemoglobin and hematocrit, the reason being unclear. It is advised to check these values after 1 to 3 months of therapy and afterward every 3 months to ensure the levels
• ‌Bosmon 62.5mg Tablet 7 ‘S can cause accumulation of extra fluid in tissue spaces (edema) of the body which should be managed by appropriate treatment modality. This is particularly of concern in patients with chronic heart failure
• If you have phenylketonuria (PKU) make sure to read the contents of this medicine to rule out the presence of aspartame
• Bosmon 62.5mg Tablet 7 ‘S must not be stopped abruptly, there should be a gradual reduction in dose over 2 weeks to allow your body to adjust properly
• Fertility issues must be discussed before starting the therapy. Bosmon 62.5mg Tablet 7 ‘S has been reported to decrease the sperm count in semen
• Ask about the amount of fluid you can take from your doctor. Patients with PAH are sometimes put on limited fluids in the diet
• If your child is taking Bosmon 62.5mg Tablet 7 ‘S, closely monitor him/her for the signs of puberty and tell your doctor once puberty starts
